Piezo is an instrument that produces ultrasonic wave from electrical energy. The ultrasound waves which have reached to a special probe cut and shape the nasal bones with small blades. The classical rhinoplasty is performed by hammer, osteotomes, and rasps. The surgeons generally use the instruments which they are most convenient for. Although Piezo causes less bruising and swelling, because of the prolonged duration of the surgery and the need of wide opening on the dorsum, to my opinion it causes an extra swelling and bruising. However, the nasal bones are mobilized with an aesthetic contour. The irregular fractures are avoided. It is possible to use the osteotomes with the same mastery as well. We like to use the new devices for the sake of ultimate rhinoplasty. But finally, let's not forget that the experience and the skill of the surgeon remain as the fundamental.
